Insurance

• I am in-network with BCBS PPO, Cigna/Evernorth, Aetna, Medicare, BCBS Community Health Plan Network (BCCHP), United Healthcare PPO, and Optum. I can also provide you with a statement you can send to your insurance carrier for reimbursement.  
• I offer sliding fees as well as out-of-network and out-of-pocket work.  
• If you have questions, I am happy to discuss your unique situation.

• Checks, credit cards, and Zelle are accepted forms of payment.
